I wish to play cricket for the Lancaster Park-Woolston Cricket Club, and subsequently I undertake to pay the appropriate subscription by 30th October 2009. I am also aware that payment received after this date will require me to pay the full subscription, (unless regular part payments have been authorised by Nathan Jones - Club Secretary/Treasurer) to be paid in full by the end of the playing season 31st March 2010. Failure to pay a subscription or have a suitable payment arrangement in place by October 30 2009 will make players ineligible for selection.

I am also aware that non-payment WILL result in my name being lodged with a Debt Collection Agency. Additional charged WILL occur.

I authorise Lancaster Park-Woolston Cricket Club to retain and use the personal details that I have given only for the purpose of the clubs administration and to inform me of any matters that the club feels may be of interest to me.

I being over the age of 18 years hereby apply for membership of the Woolston Club Inc, as I am a member of the Cricket Club. If you are not over 18 this does not apply.
